Understanding the different types of quality improvement tools will be essential in defining how to interpret or resolve an issue. In our reading we learn that there are two types of tools. Quantitative and Qualitative. Quantitative tools measure performance using graphs, pictures and other tangible data to monitor performance. During this stage, is when ideas are set and brainstorming is most useful. Part one of this question, identifying the reason for increase complaints would definitely be considered a quantitative tool situation. The best way of figuring out what is causing the issue would be to analyze the current practices within the HIM department. Out of the possible tools that could be asserted, I believe a check sheet will provide the best way of viewing what their current process it. As we learned in the past two units, the structure, process and outcome quality measure categories will provide a greater out look on what should be evaluated. I do believe this would be a process measure due to the nature of the problem.
Qualitative improvement tools are used to generate ideas, set priorities, maintain direction, determine causes of problems, and clarify processes. Qualitative tools provide a wide variety of tools that can be used to improve (health administration press, 2009). Specifically, determining the causes of problems and clarifying the processes is why I choose this to be the tool needed for the second portion of the question. A flow chart maps out what I call it as the chain of command. It shows what steps are involved and which ones to take in lieu of another. This will be a great way to clarify the processes within the HIM system. The most effective tool will be the cause and effect model. Cause and Effect charts identifies the methods, environment, people and equipment to possibly find a result to the problem, which is the ultimate goal in this situation.

The qualitative quality improvement tool I would use in this situation is the Five Why’s tool in order to analyze the current increase in complaints. The quantitative quality improvement tool I would use would be the Survey method. The idea behind Five Why’s is that after the question why is asked five times in regards to the issue, the root cause will reveal itself. Spath (2013) recognized, “The Five Whys tool helps an improvement team dig deeper into the causes of problems by successfully asking what and why until all aspects of the situation are reviewed and the underlying contributing factors are considered. The idea behind the Survey is to obtain actual numbers in regards to the complaints. A target audience should be selected, being everyone most affected by HIM. The survey should reveal a percentage of unhappy individuals in the particular area where the problem is located in addition to the number of people or processes affected. Spath (2013) states, “Surveys are instruments used to gather data or information.”
